The Connected Community Pathways (CCP) program assists those experiencing palliative care or cancer.

The program provides culturally safe and respectful services to Aboriginal and Torres Strait Islander Community members that are on a palliative care or cancer journey.

The service includes:

  • advocacy
  • care management
  • transport services
  • advanced care planning
  • promotion of cancer screening.

The Coomera Integrated Health and Wellbeing Hub is host to our new colposcopy service for women, eliminating the need for a referral to the hospital system for the procedure.

Eligibility

To be eligible for this program you must:

  • identify as an Aboriginal and/or Torres Strait Islander person, be a partner of an Aboriginal and/or Torres Strait Islander person or a parent or carer of an Aboriginal and/or Torres Strait Islander person
  • live in the Gold Coast region.

Accessing the service

Access to this program is by referral only through a GP, Health Practitioner, family/friend or a self-referral among other referral pathways.
