Kalwun's Pod Model of Care is a way of delivering health and social support services based on respect, relationships and connections, with a family centred approach.

The Pod Model is Kalwun’s commitment to ensuring our services and systems match with the way our people and community operate.

There are four Pod Teams at the Kalwun Coomera Integrated Health and Wellbeing Hub, all of which are working towards what you and your family need to be healthy, strong and deadly.

Who will be in my Pod Team?

Medical Receptionist

The medical receptionist within your Pod is responsible for connecting you with your Pod Team members, working with you to organise appointments; including transportation, processing Medicare, and performing a range of administrative tasks to support the Pod Team.

Health and Wellbeing Workers (HWW) and Aboriginal and Torres Strait Islander Health Workers (AIHW)

Your Wellbeing Workers and Aboriginal and Torres Strait Islander Health Workers connect with the other Pod Team members to support you and your family on your health journey. They will offer guidance, health education and support.

General Practitioner (GP) 

Your GP will provide overall guidance and support around the health and wellbeing of you and your family. Your GP is also there for when you are feeling unwell and you need to discuss your medical conditions.

Nurse

Your nurse will provide coordination and follow-up on the priorities you and your family have identified in your care plan. Nurses also perform procedures and provide follow-up for immunisations and dressings etc.

Social Health Care Coordinator

Your Social Health Care Coordinator will work alongside the Pod Team, providing you and your family with access to a range of services to support the social and emotional wellbeing of you and your mob.

Connecting with other health professionals

Depending on your needs, your core Pod Team members will also be able to connect you with a range of other health professionals available at your clinic — often without having to wait for an appointment with your GP.

How does a Pod appointment work?

When you call the clinic to make an appointment, the receptionist will connect you and your family with the right Pod Team member(s) and book the appropriate amount of time required for your appointment.

If you or your family require care from more than one Pod Team member during an appointment, Pod Teams will work together with you and your family to ensure privacy and smooth delivery of care.

Depending on your needs, Pod appointments can be delivered in the clinic, over the phone, via video consult or through a home visit.

Will there be any changes to the services I can access?

No, you will still be able to access all the services that are currently available through the clinic.

How is the Pod Model different?

The Pod Model supports stronger relationships between you and your Pod Team, ensuring you and your family have easier access to the right services.

With the Pod Model, you and your family will also:

  • have your care better matched to your needs
  • have the same Pod Team members working with you
  • benefit from stronger connections between you and your Pod
  • receive comprehensive care as a family.

  • Seahorse

Eligibility

The Pod Model of Care is available for Aboriginal and Torres Strait Islander people of all ages.
